Help us reduce the spread of COVID-19 by participating in this daily survey before going to work. The Sonoma County Department of Health is requesting that residents of Sonoma County self-assess symptoms related to COVID-19 each day through a survey that takes less than one minute. This initiative is part of the county's efforts to safeguard residents in both the workplace and our community.
Your participation will aid us in responsibly reopening the county as we work towards revitalizing our economy and mitigating risks. We prioritize your privacy, so rest assured that this app does not require any personally identifiable information, use location tracking, or collect any data apart from your survey responses. It is important to note that this app is exclusively intended for use by Sonoma County residents and health officials; no information will be used or shared for commercial purposes.
The app is designed for the general public, allowing Sonoma County citizens to assess whether they may be experiencing COVID-19 symptoms prior to going to work. However, it is crucial to remember that this app should not be used as a means of obtaining medical advice. The app consists of two main sections:
<ol>
<li>Resident Survey: Answer a series of questions regarding symptoms and potential contact with individuals who may have the disease. Afterwards, receive personalized recommendations based on guidelines provided by the Sonoma County Public Health department. These recommendations may include practicing social distancing, consulting with a healthcare professional, or returning to work.</li>
<li>News Feed: Stay informed with up-to-date local information related to the Coronavirus disease and its impact on Sonoma County, California. The News Feed will also offer valuable content on topics such as the proper use of face coverings, hand hygiene, and surface disinfection.</li>
</ol>
